Investigation Summary
On January 9, 2003, Employee #1 and a coworker were working to remove a four wheel drive mule from a thoroughfare. The coworker was operating a rough terrain forklift, which Employee #1 was walking behind. For some reason, the forklift backed up and ran over Employee #1 with one of its rear wheels. Employee #1 was hospitalized and treated for injuries sustained.
