Employee's hand crushed in a case sealer

On September 10, 2003, an employee was operating a taper case sealer. Her hand became trapped between the box lid and the upper lid holder as the box was grabbed by the conveyor which advanced the boxes to the taping device. Other similar case sealers of the employer, which were custom made for the facility, had guards, but a guard had not been added to this machine, a prototype model, when modifications changed guarding needs. There was nothing in place to prevent the hand from entering the pinch point, and the hand was required to be within six to eight inches of the feed point. The employee's hand was severely crushed. She was not hospitalized.
