Employee injured while reaching into press

Employee #1 was operating a hot trimming press. The press was operated by a foot pedal and the employee inadvertently stepped onto the pedal while reaching into the press to free a piece of jammed material. The press cycled and the employee's left arm was amputated at the elbow. The employee was hospitalized with his injuries. A safety device, to prevent unintended operation of the press, had been removed from the pedal.
