Employee Amputates Hand in Molding Machine

On December 11, 2002, Employee #1, a temporary employee, had been sent to another shop location while the set up was being performed and returned without the knowledge or permission of the operator. He stuck his hand into the molding machine, while the operator was performing set up Employee #1's hand was amputated, and he was immediately sent to a local hospital for treatment.
