Employee killed in cotton waste bailer machine

Employee #1 was operating a cotton waste bailer machine. The machine was a vertical motor/chain drive operated baler used to make waste cotton bales that weighed 400-500 pounds each. The employee fell through a 53-inch-long by 27.5-inch-wide by 4.5-foot-high opening. He fell approximately 7 to 8 feet and was found inside the cotton waste bailer with the ram in the down position. He received crushing injuries to his head, suffered three broken ribs, and was killed.
