Employee Amputates Fingers in Roll Forming Machine

On February 14, 2005, Employee #1 was rolling a carbon steel shell on the Lown roll forming plate bending machine, Model Number G-574, Serial Number G-5322. The Lown roll forming machine has three rollers. Two of the rollers are in-running and the other is outgoing. The rollers are 6 in. in diameter. The top roller is 76 in. in length and the other two rollers are 74 in. in length. The opening between the rollers is just a little more than 0.25 in. to allow the stock to be fed into the machine. This was the measurement of the opening at the time of the inspection. According to the company, the opening can be as large as 1 in. and can be adjusted by the operator of the machine. The adjustments could be made several times a day depending on the gauge of the material. Employee #1 was rolling 14-gauge material and had rolled a sheet of stainless first and then rolled a sheet of carbon steel. The sheets measured 17.625 ft by 15.187 ft and could vary up to 40 in. The only type of machine guarding on the equipment is an emergency stop button on the control panel of the machine and a stop bar that runs the full length of the machine. When depresses, the stop bar stops the rollers. When rolling the shell it may have rolled tighter than Employee #1 wanted. He tried to pull the part off the rollers and his gloves were pulled into the roller. He sustained an amputation of the little and ring fingers on his right hand and the ring finger on the left hand. Employee #1 was hospitalized.
