Employee Falls from Scaffold

Employee #1, working for Constructural Visions Inc. in Gretna, NE, was involved in erecting a prefabricated metal building. At the time of the incident, Employee #1 and two coworkers were installing a wood frame dividing wall. Employee #1 was standing on a Safeway scaffold, with working platform height of about 8 ft and 6 in., to secure the raised wall to the rafters while his coworkers were securing the wall to the base plate. Employee #1 fell off the scaffold and suffered a fracture to the left rear area of his skull. He was hospitalized due to his injury. The scaffold was two sections tall, each being 5 ft, had a base size of 10 ft by 5 ft and did not have guardrails installed.
