Investigation Summary
Employee #1 was preparing to place a 1,500-pound coil of steel on a splitter machine. The coil was approximately 4 feet in diameter and less than 1-foot wide. He used a Toyota forklift with a capacity of 5,000 pounds to move the coil from the storage area to the splitter machine by lifting it through the center hole of the coil. Since the splitter was already in use, Employee #1 lowered the coil to the floor near the splitter, balanced it on the narrow end, and backed the forklift away. He was standing near the coil, perhaps trying to remove remnants of a pallet, when the coil fell over and crushed his legs. He was hospitalized with his injuries.
