Machinery Activates And Traps Employee

At approximately 10:45 a.m. on August 06, 2003, Employee #1 was investigating a problem with the automated system in a warehousing operation. The automated system involves a forklift unit which was designed in Germany and installed approximately two years ago. This unit is designed to retrieve warehoused items from particular locations based on a geometric schematic which identifies locations based on coordinates. Employee #1 and a coworker accessed the forklift unit in an effort to diagnose a computer read error. The data base was indicating that a storage shelf location was empty when it was in fact full. Employee #1 and the coworker rode the unit to a height of 45 ft and Employee #1 climbed onto the storage rack in order to assess why the unit was failing to identify the pallet stored at that location. Employee #1 passed in front of a laser eye which caused the automated forks to extend and trap Employee #1's left arm between the fork lift unit and a pallet at the wrist. Employee #1 sustained a severe contusion to the left forearm. There were no other injuries associated with this accident and Employee #1 was treated and released from a local hospital.
