Investigation Summary
On November 22, 2004, Employees #1 and #2 were preparing a work platform to be removed from a slip form construction project. The employees were removing parts of a jacking system. The work platform on which the two employees were standing, gave way and the employees fell approximately 137 ft to the interior of the silo. Both Employees #1 and #2 were killed.
