Investigation Summary
On April 23, 2003, Employee #1 was helping a press operator thread new coil stock into a press. Employee #1 was standing in an area approximately 10.5 in. wide between the press and the light curtain. The light curtain did not detect Employee #1's hand in the way of the press. The press cycled and amputated two fingers. Employee #1 was hospitalized.
