Employee died when crushed by fork lift tipping over

On June 11, 2003, three employees were tearing down and removing steel racks from a warehouse. One employee was cutting the steel racks while Employee #1 moved the sections of the steel racks and tipped them over using a fork lift truck. Once the steel racks had been tipped over using the fork lift, Employee #1 and another coworker moved the steel racks, each using their fork lift truck on either side of the steel rack section. Employee #1 and the coworker would then move the steel rack section outside to a staging area for the racks to be picked up and hauled away. While Employee #1 was moving the last steel rack section, something caused the approximately 22-foot tall steel rack section to sway. The steel rack section then fell causing the fork lift to tip over, crushing Employee #1 in between the cab of the fork lift truck and the concrete floor. Employee #1 was pronounced dead at the scene.
