Investigation Summary
On October 26, 2002, Employee #1 of Forrest Davis Construction was driving a semi-tractor trailer carrying an excavator, when the semi-tractor trailer became stuck in the sand on the side of the road. He got out of the semi-tractor trailer and began walking down the road, on a downward incline, to where his coworker was working. Employee #1 explained to the coworker that the semi-tractor trailer was stuck. The coworker got in a vehicle and drove up the road to the semi-tractor trailer. Employee #1 began walking up the road toward the semi-tractor trailer. The coworker got in the excavator on the semi-tractor trailer and used the excavator to push the semi-tractor trailer free. The semi-tractor trailer then began to roll down the incline and struck Employee #1. Employee #1 pinned between the road and the trailer and was dragged down the road. Employee #1 was killed.
