Falling Platform Crushes And Kills Employee

Employee #1 was working as a contractor at a remote jobsite troubleshooting and subsequently repairing a Hanel Lean Lift retrieval system. Employee #1 was working underneath a suspended retrieval platform (extractor) weighing approximately 2000 lbs. The extractor was not locked out nor was it blocked or cribbed. When Employee #1 removed two of three gapping nuts on the electromagnetically-operated spring pressure brake which control the vertical movement of the extractor, the extractor fell on top of him pinning him underneath. Employee #1 died from his internal and head injuries.
