Investigation Summary

Investigation Nr: 201202157
Event: 04/28/2003
Two employees injured due to logging accident

At approximately 11:45 a.m. on April 28, 2003, Employee #1, a choker setter, and Employee #2, a hook tender, were working on a logging crew in Toledo, Oregon, monitoring the operation of a skyline. Employees #1 and #2 attached the skyline to the tail hold strap and stepped ahead, uphill from the skyline, giving the go-ahead signal. The line cleared through a patch of siwash rocks, and for some reason struck Employees #1 and #2 as they stood in the bight. Employee #1 was thrown up the hill and hollered for help, while Employee #2 rolled down the hill and was knocked unconscious. Both men were hospitalized and treated for injuries sustained. Employee #1 suffered a fractured arm, which required surgery and Employee #2 was pronounced dead.
