Employee dies of heart disease after fall into hot water

Employees were assigned to blend a batch of industrial textile soap in a 1,500 gallon mixing vat. Employee #1 positioned himself on a wooden pallet alongside a 55 gallon drum of Tall Oil Fatty Acid; both were raised to the top of the mixing vat by the company forklift. Once in position over the tank part, Employee #1 began attempting to pour the drum contents into the vat. Employee #1 lost control of the drum and, while he was attempting to restabilize it, both he and the drum fell into the vat, which contained approximately 1,300 gallons of 141 degree F water. A company vehicle transported Employee #1 to the hospital, where he later died of coronary disease.
