Investigation Summary
On June 24, 2003, Employee #1 was driving a tractor from one farm to another farm (miles a part). The tractor had several dead baby hogs in the front end bucket. The tractor left the road and rolled down a large embankment. Employee #1 did not have roll over protection properly in use, nor was his seat belt fasten. The tractor rolled on Employee #1 crushing him to death.
