Investigation Summary

Investigation Nr: 201953213
Event: 07/10/2002
Employee Found Dead Under Overturned Garbage Truck

On July 10, 2002, Employee #1 was operating a front loading garbage truck, Mack LE, with driving controls on both right and left side. He was working alone on a rural residential pick-up route he had always run. Once on the residential route he can operate the truck from the right side either standing up or sitting down. He was not required by the employer to use the seat belt or door strap when going from pick up points if not more than half mile. The controls will not allow the truck to go more than approximately 20 mph when operated from the right side. He was found dead, pinned under the trucks rear mud flap frame after the truck had flipped over after running off the right side of the road.
