Investigation Summary
On September 11, 2002, Employee #1 was eating lunch in a doorway across the street from a construction site. A gust of wind blew down some wooden forms that were supporting the deck on top of the approximately 800 ft tall structure. The falling wooden forms struck Employee #1 on the head, killing him.
