Employee killed when run over by wheel loader

On December 9, 2002, Employee #1, a lead person, instructed an operator of a Caterpiller wheel loader to back up from the end of the pit area. The wheel loader was being used in the manual collection of debris from a pit scale area. As the operator began to back up, Employee #1 fell head first from a raised side scale wall toward the rear of the front driver side wheel. The operator could not stop the wheel loader and ran over Employee #1. Employee #1 was pronounced dead at the scene.
