Investigation Summary
At approximately 10:15 a.m. on January 2, 2003, Employee #1 was working in a vegetable farm. Employee #1 was run over and crushed by a truck. At the time of the accident, Employee #1 was picking bell peppers at the edge of one of the "drive middles" of the farm. The term "drive middle" means a narrow lane running between blocks of bell peppers where vehicles drive through. Employee #1 was killed.
