Employee Fractures Pelvis in Slip and Fall

On November 11, 2005, Employee #1 was walking out of the planogram room, when she stepped on several 5-inch by 5-inch advertisement postcards on a signing board that was on the floor. The glossy surface of the board caused her to slip and fall. She fractured her pelvic bone, underwent surgery, and was hospitalized.
