Employee Is Killed When Crushed by Truck

On September 6, 2005, Employee #1 was working at a business that repairs and maintains shipping containers. The employer services several yards and has a designated service area within the yard where repair and maintenance work is conducted on site. Employee #1, a supervisor, instructed a mechanic to bring an empty 40-ft container from the north yard to the service area so it could be repaired. Several containers were already parked in the service area, waiting to be repaired. The area did not have sufficient clearance for the turn, given the swing radius allowance that the tail end of the container must have to manipulate the turn successfully. As the mechanic turned the yard hustler to park it in the designated area, he heard a "noise." The mechanic stopped the truck and inspected the rear of the container. The mechanic found Employee #1 lying down on the ground, unconscious. Employee #1 died.
