Employee Amputates Finger Caught Between Wire and Material

On January 21, 2005, Employee #1 was working in a galvanizing plant. Materials that are to be galvanized are tied to racks which are picked up by cranes to move them to the different staging areas including the dip tank. Prior to dipping, the materials are tied to hooks which are welded to wide angle flanges. These wide flanges are attached to the rack by an eye-plate. A 1-in. diameter steel pin goes through this eye-plate and to the prepunched hole in the rack. Due to the dipping process, the rack and its components are exposed to deteriorating agents and must be inspected on a regular or periodic basis. Employee #1 was adjusting the tie wires of approximately 24 pieces of wire mesh that was already suspended on a hook. While standing on a ladder, Employee #1 was trying to pull 4 pieces of wire mesh downward to keep them from sticking to the other pieces. While Employee #1 was adjusting the tie wires, the load shifted, tightening the knot on the tie wire. Employee #1's right index finger was caught between the tie wire and the material and severed the right index fingertip. Employee #1 was treated for his injuries but not hospitalized.
