Employee Injured Finger in Cutting Machine

At 5:30 p.m. on August 16, 2004, Employee #1, a primary employee of a temporary staffing agency Select Personnel Services at a wire products fabricating plant of Cove West, was assisting another Select Personnel temporary worker in guiding and feeding the wire during setup by putting his gloved hands into the feeder block point of operation of a Nilson/US Baird wire forming machine Serial number 43-2101, shop machine #5. The operator jogged the machine thinking Employee #1 was clear, and the block rapid traversed, catching Employee #1's gloved right middle finger between the block and the feeder stop, amputating the fingertip before the first digit. Employee #1 was taken to Norwalk Medical Center hospital where part of the bone of the first digit was removed, and then the finger was sutured and dressed. Employee #1 was discharged the next day.
