Investigation Summary
At approximately 4:30 a.m. on October 31, 2002, Employee #1, a coating line operator, was working full-time for a cold roll steel electro-galvanizing company. Processing of cold rolled steel consists of the application of a zinc coating onto cold rolled steel of varying lengths and thicknesses. The coating process takes place over a 210-foot, two-tiered line. The electro-galvanizing process involves the use of a series of acid baths (6 foot by 10 foot vats) using sulfuric acid. The cold rolled steel is also treated with various other chemicals, such as chromic, phosphoric and hydrofluoric acids to further protect the steel. The cold rolled steel is un-wound, treated and re-wound. Employee #1 and a coworker discovered a "ping" (flaw) on the coated product. They went searching for the origin of the "ping" marks by observing the quality of the product as it left each series of rollers, including black rubber rollers and contact rollers. Black rubber rollers, which were approximately 6 feet long by 16 inches in diameter, receive the cold rolled steel from the chemical baths and feed it to contact rollers, which are metal and are approximately 5 feet long and 20 inches in diameter. At the time of the accident, the line was running approximately 140 miles per hour. After discovering the location of the flaw, Employee #1 and coworker were trying to remove the foreign object from the roller that was causing the "ping". They were working on the second tier of the two-tiered coating line. The coworker tried to remove the foreign object from the Number 7 roller with sandpaper on a stick. When it was evident that this wasn't removing the problem, the coworker told Employee #1 that he was going to contact the control operator so the line could be slowed down. After his coworker left the area, Employee #1 tried to remove the flaw with a 12-to-16-inch file. Employee #1's hand was caught and his finger was amputated. He was hospitalized.
