Materials Strike And Kill One Employee And Injures Another

Two employees were working from a locally fabricated, 16-ft by 4-ft work platform, with welded guardrails that was attached to the forks of an all-terrain Caterpillar powered industrial truck. The work platform was being positioned approximately 15 ft above the ground and at a 40-degree angle to the industrial truck, in order to place eight, 3-in. by 8-in. by 30-ft, steel purlins on the steel beams of a structure being constructed. The steel purlins were being carried on the guardrails. The boom was over-extended and descended due to the excess load capacity and the angle of the extension. The material on the guardrail struck the employees. Employee #1 suffered a fracture and died, and Employee #2 suffered bruises, contusions and abrasions and was hospitalized.
