Employee killed after falling from raised pallet

On July 18, 2003, Employee #1 was working for a service company moving office equipment from one warehouse to another. He was standing on a pallet with a file cabinet while it was being elevated about four feet by a fork lift. When the cabinet began to topple over he lost his balance and fell, striking his head on the concrete. Employee #1 was killed. The employee was born in Mexico and his primary language and the primary language used on the worksite was Spanish.
