Investigation Summary
On May 21, 2002, Employee #1 was located at the top of a water tower to make preparations for painting. To reach the top of the tower, he rode a lift chair to a fixed catwalk of the water tower and then climbed to the top through the use of a ladder. As Employee #1 was on the water tower and a coworker was half-way up the tower in the lift chair, the ladder that he used to access the top fell off the roof. Approximately five seconds later, Employee #1 fell to the ground. Emergency services were called and coworkers on the ground performed CPR until Employee #1 was transported to Lake Point Medical Center, where he was pronounced dead.
