Investigation Summary
On October 9, 2002, Employee #1 was operating a Flexo Folder Gluer machine, feeding sheets of cardboard into the machine when his right hand got caught in the rollers. The bottom guard on the machine, which measures 8 feet across the face, was located 1.5 inches above the table with the rollers located 1 inch behind this guard. Employee #1 received severe injuries to his right hand with tendon and ligament damage and was hospitalized.
