Investigation Summary
On or about April 22, 2003, Employee #1 was crushed to death by a coil of steel. He was attempting to retrieve a rubber shim that had been jammed between the 4500-lb steel coil and the payoff mandrel to allow the payoff to continue turning. Employee #1 had collapsed the mandrel to retrieve the shim, when the coil became dismounted, striking and killing him.
