Investigation Summary
Employee #1 was operating an X221 Gel Filling and Assembly Machine. Employee #1 reached into the machine while it was running to remove a deformed tube from in front of the supply shoot. The carriage for the tube supply came down on Employee #1's finger resulting in the amputation of the tip of left index finger. Employee #1 was treated for his injuries but was not hospitalized.
