Employee Is Killed in Fall When Elevated Box Overturns

On August 26, 2002, Employee #1 and two coworkers were using a three-sided wooden trash box elevated by an all-terrain forklift to wash a brick wall. At one point, they all went to one side of the box and it tipped off the forks. Employee #1 and the two coworkers were ejected from the box and fell approximately 10 ft to the ground. Employee #1 died at the scene of injuries sustained in the fall. The trash bin was not secured to the forks.
