Investigation Summary
On July 16, 2002, an employee drove the compactor out of the covered trench, began to climb and steer towards a diesel truck for refueling. As he was steering, pivoting and turning to the right, the front roller was climbing up a sloped of approximately 18-in. rise, rough and irregular soil surface. The compactor rolled over approximately 9 ft from the edge of the covered trench. The employee received lacerations of the scalp and head trauma and was killed.
