Event: 04/11/2002

Employee killed while over-pressurizing drum

On April 11, 2002, Employee #1 was attempting to remove a solution of sodium hydroxide and sodium azide from spent activated carbon-filter drums prior to disposal. Apparently he pressurized the drum with a compressed air line set at 110 to 120 psi to force the solution up and out of the drain hole. The over-pressurization caused the drum bottom to separate. The drum top and sides struck Employee #1 in the torso and face, killing him.
