Investigation Summary
On June 25, 2005, an employee was working on Labeling machine, Label Transfer System, model Digitran 201 cd, manufactured by Autotec Digital AG. This machine printed graphics and text on a plastic film and heat presses it onto the compact disc. After the preliminary setup, the normal operation of this machine was completely automated and the doors were interlocked, which provided adequate protection for the operator. At the time of this accident, the machine was making some unusual noise due to some loose part. As an operator of this machine and being the most experienced person with the machine, the employee was trying to pinpoint the source of this noise. Because of the somewhat dark tint on the glass enclosure and the doors of this machine, she decided to disable the door interlocks and started the machine with the doors open. In order to better listen to the noise, she placed her left ear near the moving printing head of the machine. She also had her right hand next to her face. Accidentally, her right middle finger went under the printer head resulting in the amputation of the finger at the first knuckle. The employee amputated about an inch of her right middle finger. She was treated in the emergency room and was sent home after about 6 hours.
