Investigation Summary

On April 24, 2003, Employee #1, a falsework bent positioner, was involved with the construction of an elevated roadway. A Link Belt lattice boom crawler crane was lifting and substantially positioning a bent that weighed several thousand pounds. Employee #1was standing about 1.5 ft from the bent bottom, and pushing one end of the bent about eight ft from the bent end with his extended arms, with his forehead at the same level as the bottom of the bent. The crane operator lowered the suspended bent about one foot to allow Employee #1's arm to extend horizontally without an upward pitch, and thus be in a more favorable position to maneuver the heavy metal bent. The bent moved toward the employee and struck his forehead. Employee #1 was hospitalized with vertebrae and disc injuries to his neck and back.
