Investigation Summary

At approximately 12:05 a.m. on April 11, 2003, Employee #1 was operating a Raymond Reach Truck forklift, Model Number EASI-R3OTT/EZ-A-01-21230, in a warehouse. There was some juice spilled on the floor that was mopped up and cones were placed in the area to warn the forklift drivers of the wet conditions. Employee #1 was driving through the wet area when the Reach Truck started to skid towards a pole during a turn. He stuck his foot out of the Reach Truck and his left foot was crushed between the pole and the Reach Truck. He suffered a left heel avulsion and was transported to St. Jude Medical Center in Fullerton, CA for treatment
