Investigation Summary

At approximately 8:45 a.m. on April 18, 2003, an employee of Ewles Materials was walking along the backside of the crusher. The loader operator was dumping a load into the hopper. A large chunk of asphalt rolled off the left side of the bucket and hit the top guardrail that runs along the top of the discharge side of the feeder. It continued traveling and hit a back guardrail and then fell off the backside of the crusher and struck the employee on the head, back and left side. The employee was approximately 30 ft away from the initial point of impact. The employee sustained fractured ribs and vertebrae. He was hospitalized for over 24-hours.
