Employee's finger amputated by ladder

On April 2, 2003, Employee #1, of CellTech Manufacturing CA. Inc., a pharmaceutical producer, was monitoring a processing machine. He was sitting on a partially folded two-rung stepladder that was leaned against a large fixed plastic storage tank. While he was checking a temperature gage, his right hand was hanging down in-between the two hinged parts of the stepladder. The stepladder shifted and the fingertip of Employee #1's right ring finger was caught when the stepladder folded tight. Approximately 0.5 in. (including some bone) of the fingertip was amputated. Employee #1 was not hospitalized.
