Investigation Summary
On March 26, 2003, Employee #1, working for an employer engaged in the warehousing and sale of hardware and lumber products, was picking scrap paper and other debris near a railcar parked next to the lumberyard. A coworker was operating a forklift manufactured by Yale, Model Number GLP100MGNSBV098, Serial Number B813D1543, with a capacity rating of 10,000 pounds. He was unloading stacks of plywood material from the railcar and was backing up the forklift when the right rear wheel of the forklift ran over the left foot of Employee #1. Employee #1 suffered five bone fractures in his left foot and was hospitalized.
