Investigation Summary

Investigation Nr: 200101590
Event: 03/14/2003
Employee crushed to death by a trailer

On March 14, 2003, Employee #1, of Ferrous Processing and Trading Company, was assisting a coworker in moving the tractor-trailer away from the open bay door. He moved to the rear of the trailer and began signaling with hands for coworker to move the tractor-trailer back. The coworker who was viewing Employee #1's hand signals through the back windshield of his truck began backing up slowly. The coworker said while he was backing up the tractor-trailer and watching Employee #1 through his back window of the truck, Employee #1was waving him back and then he signaled with his hands for him to stop. At this point the coworker attempted to stop the tractor-trailer by placing his foot on the clutch pedal. When he did this, his foot slipped off the smooth metal clutch pedal causing the tractor trailer to jerk backwards in reverse pinning Employee #1 between the back of the trailer and the two tier cider block pillar that was located between the two bay doors. The coworker immediately pulled the truck forward releasing Employee #1 from between the trailer and wall. The owner of the facility was standing by the junk cars in the front of the facility at the time of the accident and witnessed the tractor trailer jerking back in reverse and pinning Employee #1. The owner called the emergency medical services for help and went to the street in front of the facility to signal rescue units upon their arrival. The coworker stayed with Employee #1 until police and paramedics arrived on the scene. Employee #1 later died at the Metro Health Trauma Center.
