Investigation Summary
On April 12, 2002, Employee #1 was on an 11-ft-high scaffold not fully planked and unguarded, removing a piece of ceiling. As he extracted parts of the ceiling, a section of ceiling fell and hit him. The force of the debris knocked him off balance and caused him to fall. He fell 11 ft and struck the concrete floor. Emergency medical personnel was summoned. They arrived and transported him to the hospital. At the hospital, he received stitches for a laceration on his head and medical care for several contusions on his leg. Upon completion of these medical procedures and treatment, he was released from the hospital.
