Employee Killed When Struck by Falling Chemical Drums

At 11:17 a.m. on June 17, 2002, Employee #1, a driver for A A A Cooper Transportation in Chicago, IL, was making a delivery at the Bellwood Water Department. He backed his 40 ft- semi-trailer into the parking lot. Then, he used a walk-behind forklift to move a pallet of drums containing water treatment solution onto the trailer's hydraulic lift gate. As he was struggling to pull the pallet off the jack onto the lift gate, Employee #1 lost his footing and fell to the ground. Two drums filled with solution fell on top of him. Employee #1 was killed.
