Employee injured when struck by falling cage

At 7:00 p.m. on June 4, 2002, Employee #1 was operating a powered industrial truck, a Raymond order picker, inside the freezer as part of his normally assigned duties. He was in the process of picking an order and had elevated the industrial truck and was traveling in reverse down an aisle with an obstructed view due to the cage attachment on the forks. The cage struck a pallet that was sticking out beyond the edge of the storage racks. The cage was not secured and it fell forward, striking Employee #1 and pushing him into the operator's panel. As a result, Employee #1 experienced periods of unconsciousness, lacerations to the forehead and back of the head, lacerations to the upper lip requiring stitches, a bloody nose, and head, neck and back trauma with possible spinal damage. Employee #1 was hospitalized.
