Investigation Summary
On October 28, 2003, Employee #1 and Employee #2 were installing siding on a ladder jack scaffold at a height of 14 feet. Employee #1 was having trouble locking a piece of siding into another and in an attempt to get the siding into place, Employee #1 pushed and lifted the piece with a strong, sudden movement. This movement caused the ladder to move away from the building and fall backwards. Employees #1 and #2 fell with the scaffold to the ground. Employee #1 suffered bruising and Employee #2 suffered a fractured hip, requiring hospitalization.
