Investigation Summary
On April 8, 2003, Employee #1's clothing became caught in the moving roller parts of a steel straightening machine and Employee #1 was thrown onto the concrete floor several times. The Elyria, Ohio Fire Department extracted Employee #1 from the machinery, and he was transported to the EMH Regional Medical Center where he was pronounced dead at approximately 10:00 p.m. on April 8, 2003. Investigation revealed that the company requires employees to feed round steel bar stock into a straightener by hand,. When the bar stock enters the point of operation, it is immediately sped up to 1200 to 2400 rpm. Employee #1 inadvertently contacted the rotating steel bar with his left leg, causing his clothing to wrap around the bar and throwing him onto the floor forcibly on his head. Employee #1 sustained a neck fracture which was instantly lethal.
