Investigation Summary
On June 10, 2002, Employee #1 and two other coworkers were operating a 500-ton revolution mechanical press for stamping operations. There were no safeguards in place, which presented risk to workers during the press cycle phase. Employee #1 was reaching into the die area, located between the light curtain and the press frame, when one of his coworkers cycled the press. Employee #1's right arm and left hand got caught between the platens. Employee #1 was hospitalized and treated for his injuries, which included amputation.
