Investigation Summary
On May 22, 2003, Employee #1 and coworkers were working to felled several trees. Employee #1 was holding onto a rope tied to one of the trees, to help guide it down to a pre-selected location in the hazard area. The tree started to fall and Employee #1 bumped into another employee. This prevented him from exiting the hazard area in time and he was struck by the falling tree. Employee #1 was hospitalized and pronounced dead due to injuries sustained.
