Employee Falls From An Unsecured Ladder

On October 15, 2001, Employee #1 was using a Louisville fiberglass extension ladder 9 feet, 2 inches above a steel plate floor. Employee #1 was emptying scrap from an open-sided trash bin raised up over the edge of a 20-yard dumpster by a forklift. No witnesses observed the incident. A sales representative for Harpo Investments, Employee #1's employer, found Employee #1 on top of the ladder. The representative noticed skid marks, which suggest that the base of ladder slid out. The ladder's top and bottom were not secured. Employee #1 had a skull fracture, left arm and hip fracture, and bleeding inside his head. Employee #1 died 1 month after the fall in a rehabilitation center from blood clots in his lungs, possibly from deep vein thrombosis, secondary to hip fracture and multiple surgeries.
